Patient Data & Consent Notice

Last updated: 7 November 2026

This notice is intended for people submitting website enquiries and patients receiving clinical care at HairDoc. It explains specific categories of information collected during the enquiry and care process. It should be read alongside our detailed Privacy Policy.

1. Website Enquiry or Appointment Form

“HairDoc, operated by DR SATESSHS HAIR CLINIC & RESEARCH CENTER PRIVATE LIMITED, collects the name and phone number you submit to respond to your enquiry; the hair/scalp concern and preferred branch you select to route your request; and any appointment or callback preference to arrange a consultation. We need contact information to respond to your request. If you proceed with care, a separate intake process may collect additional clinical information. For permitted access, correction, erasure or consent-withdrawal requests, or to make a privacy grievance, email info@hairdoc.in with ‘HairDoc Privacy Request’. See our Privacy Policy for further details. Submitting an enquiry does not automatically opt you into unrelated promotional messages.”

2. Clinic Intake — Patient Information Notice

“For assessment or treatment, HairDoc may collect your symptoms and hair/scalp concerns, relevant health and medication history, examination and imaging findings where appropriate, clinical notes, recommendations, session details and follow-up records. We use these records for assessing your concern, providing care, monitoring progress, maintaining necessary records and meeting applicable duties. The information may be accessed by treating staff and, where needed to operate our systems, service providers. For privacy requests or grievances, email info@hairdoc.in. See our Privacy Policy.”

Detailed medical information is generally collected during in-clinic consultation. Before clinical procedures or clinical photography, HairDoc explains the relevant process and records the patient’s consent on a signed paper form. When a child attends with a parent or guardian, the clinic obtains the appropriate guardian involvement and consent. Treating doctors use clinical notes and session photographs to review treatment progress.

3. Clinical Photography and Publicity Permission

HairDoc may take hair and scalp photographs during treatment sessions to review progress. Clinical consent is documented on a paper form signed in the clinic by the patient or the appropriate parent or guardian. Consent to clinical photography does not cover advertising or public display.

Before identifiable images, testimonials, patient stories or videos are used publicly, separate, voluntary and recorded permission is obtained, specifying the intended material and channels. Permission for publicity is optional and not required to receive clinical care. A patient may contact info@hairdoc.in about withdrawal of future optional publicity, subject to applicable limits.

4. Privacy Rights, Withdrawal and Contact

For applicable access, correction, updating, permitted erasure, consent withdrawal or grievances, email info@hairdoc.in with “HairDoc Privacy Request” and an appropriate identifier. The clinic may reasonably verify a request, and some records may need to be retained under medical, financial or other law. If you are not satisfied with our response, you may use any complaint route available to you under applicable law.

5. Website Enquiries and In-Clinic Consent

Submitting a website enquiry is not the same as consenting to a clinical procedure. HairDoc records consent for in-clinic care on a signed paper form. The website separately provides information about enquiry-data use and any optional marketing permissions. To ask about your clinical consent record or a privacy concern, contact info@hairdoc.in.
